We have all been there....

There is a tight schedule. The Project Manager states we do not have time to regression test the patched build.

Or

Everyone is waiting for the testers to complete their manual clicking through the application's functionality and someone states, "We need to automate all our tests!"

Or

The build engineer is compiling the candidate build for release and absent mindedly forgets the manually patched files from the previous build.

We have all been there and hopefully all learned from those mistakes, but if not...
